Cancellation Policy

-Have a valid photo ID (driver’s license, state ID, or passport) to verify your identity. -Have any required paperwork or authorization forms from your employer or testing agency. -Please be well hydrated, but avoid excessive water intake, as it may result in an invalid or diluted specimen. -If this is a follow-up, return-to-duty, or random test, please have the appropriate documentation ready at the time of collection. -After booking PLEASE contact me at my phone number 657-566-2588 so we can deposit and appointment. -A customized deposit will be sent via invoice to secure your appointment -Additional fees may apply based on mileage, test type, age, and location. You will be notified of your remaining balance via email once your requisition is received. -Please notify Concierge By NRC at least 48 hours in advance if -you need to cancel or reschedule so that other clients may be accommodated. To cancel, call (657)-566-2588 and leave a detailed voicemail, or reply to your original conformation email and SMS. -All deposits are NON-REFUNDABLE if appointments are NOT canceled within the 48 hours. -In the event of cancellation by the patient the deposit will be retained. Concierge by NRC mobile services does not bill insurance providers directly. -Clients are responsible for submitting claims to their own insurance company for possible reimbursement using the insurance codes. -Insurances codes:        36415    36416     99195​
