Terms & Conditions

Cancellation Policy

  • Please notify Concierge By NRC at least 48 hours in advance if you need to cancel or reschedule so that other clients may be accommodated.

  • To cancel, call (657)-566-2588 and leave a detailed voicemail, or reply to your original conformation email and SMS.

Refund Policy 

  • All deposits are NON-REFUNDABLE if appointments are NOT canceled within the 48 hours.

  • In the event of cancellation by the patient the $100 deposit will be retained.

Payment Terms

  • $100 convenience fee deposit is required to secure your appointment & goes towards your total balance.

  • Additional fees and may apply based on mileage, test type, and location. You will be notified of your total balance via email once your requisition is received. 

Client Responsibility

  • It is the client’s responsibility to provide accurate health information, lab requisitions, and instructions from their ordering provider.

  • Clients must follow all pre-test instructions (including fasting, hydration, or medication requirements) as directed by their physician or ordering provider.

Service Limitations

  • Concierge By NRC Mobile Services provides specimen collection and point-of-care testing services only. We do not interpret results, diagnose medical conditions, or provide medical treatment.

  • All test results will be sent directly to the ordering provider or designated laboratory for review and interpretation.
     

Liability Waiver

  • By using our services, the client acknowledges that mobile phlebotomy carries minimal risks such as bruising, dizziness, or fainting. Concierge By NRC Mobile Services and its staff will not be held liable for adverse reactions beyond standard care practices. 

  • A waiver will be needed filled out at the beginning of your first boking.

  • Once specimens are collected and transferred to the designated laboratory, we are not responsible for delays, handling, or processing performed by third-party labs.

Consent to Services

  • Scheduling and paying a deposit serves as acknowledgment that the client understands and consents to specimen collection, handling, and submission as outlined in our policies.

  • Clients may request a copy of these terms at any time for their records.

Insurance

  • Concierge by NRC mobile services does not bill insurance providers directly.

  • Clients are responsible for submitting claims to their own insurance company for possible reimbursement using the insurance codes.

Minors

  • A Parent/Guardian is required to be present at the appointment the whole time services are being provided. 

  • A Parent/Guardian & client are required to fill out a consent form at the beginning of appointment.
