Cancellation Policy

-Please fast for 6–8 hours prior to your scheduled blood draw (unless otherwise instructed by your provider). -Email a copy of your lab requisition/order to conciergebynrc@gmail.com before your scheduled visit. -For minors Receiving services, a guardian/parent must be present the whole time service is being provided. A consent form will be required to fill out at the start of the appointment. -Stay well hydrated—drink plenty of water prior to your appointment to ensure a smoother collection. -A $100 fee deposit is required to secure your appointment. The $100 goes towards your total balance. -Final payment is due at the time of your appointment. -Additional fees may apply based on mileage, test type, age, and location. You will be notified of your remaining balance via email once your requisition is received. -Please notify Concierge By NRC at least 48 hours in advance if -you need to cancel or reschedule so that other clients may be accommodated. To cancel, call (657)-566-2588 and leave a detailed voicemail, or reply to your original conformation email and SMS. -All deposits are NON-REFUNDABLE if appointments are NOT canceled within the 48 hours. -In the event of cancellation by the patient the $100 deposit will be retained. -Concierge by NRC mobile services does not bill insurance providers directly. -Clients are responsible for submitting claims to their own insurance company for possible reimbursement using the insurance codes. -Insurances codes:        36415    36416     99195​
